I have always waited in the RETURNS line for my price adjustment. You mentioned that there is a special line for price adjustments, is it labeled as such or is it just the customer service lines that are not specifically for returns?
How do I get a price adjustment when you purchase at the warehouse? I read if you purchase at the warehouse you have to go to the counter to get the adjustment I don't know about the form. I have original receipts.

I think the price adjustment form you referenced is for online orders only. Because I bought the item in-store I had to go to that warehouse to request a price adjustment. They would not give it to me without the receipt which of course is the one Costco receipt I could not find. (Someone told me you didn't need a receipt to get a price adjustment…. that they could look it up.) So my first price adjustment attempt was a fail.
